HISPC Phase 2
Purpose
In Phase 2, states were asked to implement the proposals developed during Phase 1.
In Phase 2, each subcontractor was required to:
- Develop an implementation work plan based on the proposal developed in Phase 1.
- Participate in multi-state and/or regional collaborative meetings.
- Prepare a final report describing the progress made on the implementation plan, issues encountered during implementation, lessons learned, and plans for continuing the project. The report also should include an impact analysis of work done by the state that details the changes and outcomes resulting from the HISPC project from its inception through December 31, 2007.
- Prepare a collaboration strategy with members of the collaborative work groups.
The NC HISPC team was also required to produce the following:
- A Privacy and Security Awareness and Outreach Toolkit for Public Policy Makers
- A Privacy and Security Awareness and Outreach Toolkit for Consumers
- A NC Consumer Advisory Council on Health Information Program Toolkit
- A draft model consent form for North Carolina
- A draft model data exchange form for health information exchanges in North Carolina.
